Enrollment:
Our CFLC staff would be happy to assist all families with the enrollment process. All necessary information and forms are available in the office. CFLC has an open door policy which means that no appointment is necessary for a tour. Center tours are given Monday thru Friday 6am to 11am and 2:30pm to 6pm. Tours are not given between 11am and 2:30pm because children are eating lunch and napping throughout the building.

Checklist for choosing child care

Forms for existing CFLC families:
Vacation/Sick Form: This form is to be filled out if your child will be out of school for an entire week. The proper credit will be applied to your account after the form is turned into the office.

Potty Trained Form: This form should be filled out when your child is fully potty trained. Please turn the filled out form into the office.

Change of Information Form: This form should be filled out if any of your personal information has changed. Please turn the filled out form into the office.

Physical/Shot Record Form: This form should be filled out by your child’s physician whenever necessary.
